The University of Saint Louis Elementary Department welcomes
all pupils who meet the admission requirements:
- He/she meets the age requirement.
- He/she fully subscribes to the objectives, policies, and Catholic orientation of the school.
- He/she respects and follows the religious instructions and activities in the school.
- He/she is willing to participate in all school activities.
- He/she is of good moral character

The Department reserves the right to refuse admission,
readmission or enrollment of a pupil for such reasons as:
- Limited school facilities
- Applicant’s poor academic qualifications or performance
- Applicant’s lack of moral or behavioral fitness
- Applicant’s own orientation that may run contrary to the school’s Catholic orientation
- Applicant’s lack of ecumenical bond with the Catholic Church
- Applicant’s parents’ deliberate or advertent lack of cooperation and collaboration with the school, and
- Other conditions as may be stated in the succeeding provisions of this handbook.
- Only applicants who have complied with all the requirements shall be scheduled for interview and testing/assessment.
- * There will be no interpretation of test scores in terms of description will be made. The school shall have full rights to test results that shall be kept confidential but may be released to the pupil concerned, his/her parent or guardian.

Registration
Interested applicants must register and submit the following:- Photocopy of Baptismal Certificate of the Child
- Photocopy of NSO Certificate of Live Birth
- Photocopy of Marriage Contract of Parents, if available
- Photocopy of Hearing Test Result, if available
- Two (2) copies of the most recent colored ID picture (2x2) with white background
- Interview with the DKLC Head or representative

- Parents should come for enrollment on the scheduled dates.

Registration
- The child must be (4) four years old by the end of October as per DepEd Order No. 47, s. 2016.
- Interested applicants must register and submit the following before taking the assessment:
- Photocopy of NSO Certificate of Live Birth
- Testing fee receipt
- One (1) copy of the most recent colored ID picture (2x2) with white background

Enrollment
Documents to be submitted during the enrollment:- Notice of qualification for enrollment
- Three (3) additional copies of the child’s most recent colored ID pictures (2x2) with white background.
- Photocopy of PSA Birth Certificate
- Photocopy of baptismal Certificate of the child, if Catholic
- Photocopy of Marriage Contract of Parents, if available

Registration
- The child must be (5) years old by the end of October as per DepEd Order No. 47, s. 2016
- Interested applicants must register and submit the following before taking the entrance examination:
- Photocopy of NSO, Birth Certificate of the Child
- Submit testing fee receipt
- One (1) copy of the most recent colored ID picture (2x2) with white background
Documents to be submitted during the enrollment:
- Notice of qualification for enrollment
- Three (3) additional copies of the child’s most recent colored ID pictures (2x2) with white background.
- Photocopy of PSA Birth Certificate
- Photocopy of baptismal Certificate of the child, if Catholic
- For incoming Grade I from USL Kinder pupils must pre-register as perschedule to reserve a slot. Pre-registration fee and entrance exam fee are non-refundable. Pre-registration fee will be inclusive in the Grade I school fees.

Enrollment
- The child must have pre-registered and taken the entrance examination.
- The child must have been interviewed by the Principal or her representative.
- Submit the result of the test and additional two (2) copies of the most recent colored ID picture with white background.
- Follow the enrollment procedure on scheduled date.
- For incoming Grade I from USL Kinder pupils must preregister as per to reserve a slot. Pre-registration fee and entrance exam fee are non-refundable, pre-registration fee will be inclusive in the Grade I school fee.
- They must enroll during the scheduled enrollment days. Failure to enroll on scheduled dates would mean the forfeiture of the slot.
- Submit their report card (form 138) and 2 copies of recent colored ID pictures (2x2) with white background.
- For pupils who have disciplinary cases or academic deficiencies, they must see the Principal before enrolling.
- For Late enrollees: A late enrollee is a pupil who enrolls after the scheduled date of enrollment and such shall pay a charge of P 200.00 at the Accounting Office.
Transferee for Grades I – VI will be accepted, provided the
applicant has fulfilled the following requirements:
- Submit photocopy of Form 138 marked eligible for Grade level approved for.
- General average must not be lower than 85% with no failing grades except for DKLC average must not be lower than 80%.
- Passed the entrance examination for transferees
- Transferee is of good moral character.
- Recommendation letter from the Principal of the school last attended
- Has not attended more than two (2) schools prior to transfer to USL – Elementary School Department
- Submit:
- photocopy of Baptismal Certificate of the child,
- photocopy of NSO Certificate of Live Birth,
- photocopy of Marriage Contract of Parents, if available,
- Receipt of Pre-registered Testing Fee,
- Two (2) copies of the most recent colored ID picture (2x2)
- Has the normal age for the grade level where pupil is seeking enrollment (i.e., Grade II – 7 year-old)
- Parent or guardian has been interviewed by the Principal and has presented a letter stating the reason for the transfer of the child.
- Willingness of the parents and the applicants to abide by the rules and regulations of the school.
- A pupil who discontinued his studies and who did not obtain
transfer credentials, or its equivalent or certificate of
transfer, or transcript of records (Form 137-A) from the
school shall be admitted if he meets the following
requirements:
- submission of original copy of report card (form 138) for the preceding school year, marked eligible for admission to the curriculum year in which the pupil is supposed to enroll, duly signed by the Principal, not previously marked by any other institution.
- was never subjected to serious disciplinary measures during his attendance in the school
- If due to illness, pupils will present medical certificate of fitness to study.
Withdrawal of Enrollment
- Any pupil desiring to discontinue his studies during the schoolyear must notify the Principal. The petition to drop shall take effect on the date of approval of notification to withdraw enrollment by the Principal. Parent / Guardian must fill out the dropping form and submit the same to the Principal’s Office. Failure to submit the form, the parent/guardian/pupil shall not be entitled to any refund within 1 week and the same shall be forfeited in favor of the school.
- A pupil dropping from the school after enrollment shall be charged for his/her fees in accordance with the Manual of Regulations for Private Schools, Article XIII Section 66: When a pupil registers in a school, it is understood he/she is enrolling for the whole year. A pupil who withdraws in writing will be charged 10% of the total amount due if he withdraws within the first week of classes or 20% if within the second week of classes regardless whether or not he/she has actually attended classes.
The pupil may be charged all the fees in full if he withdraws anytime after the second week of classes. However, if his/her withdrawal is due to justifiable reason (i.e., serious illness), he/she should be charged the pertinent fees only up to the last month of attendance.
- The down payment of new pupil/transferee who drops from the school year after enrolment shall be deemed non-refundable.
Securing of Transfer Credentials The pupil is issued transfer credentials which include Form 138 (Report Card), transfer eligibility and Certificate of Good Moral Character. Upon request, Form 137 is forwarded directly to the school to which the pupil will transfer. |
